If any PSIA-AASI member gets hurt while participating in covered activities, Spot’s policy can cover up to $25,000 of their out-of-pocket medical bills, with $0 deductible. Coverage works whether you have health insurance or not, and coordinates with any existing health plan you may have.

If you have an accident, focus on your injury first and get the treatment you need. Go to any licensed physician, hospital, ER, or urgent care clinic that is convenient to you. Whatever your physician recommends as treatment should be covered, whether that’s diagnostic tests, scans, surgery, specialist visits, prescriptions, procedures, hospital stays, ambulance or air ambulance rides, physical therapy, and more.

If you have a change in employment

Get documentation.

If you’ve been involuntarily transferred, terminated, or laid off from your employer ensure you keep the documentation.  If your military leave has been revoked or you’ve been reassigned obtain the official notice as it will be required when you file your claim.

First you’ll report your accident and the injuries you sustained. Then you’ll file a claim by telling us about any medical expenses you have incurred as a result of the injury.

Coverage is underwritten and claims are serviced by Mutual of Omaha Insurance Company and facilitated by Spot.
 

The first and last name should represent the individual registered as a PSIA-AASI member. If you are signing a minor up for the policy please include their name inside the form.
